Our firm has prepared this useful tool which we have labeled Family, Financial and Digital Diary which allows the user to record account information, contact information and final wishes.  Regardless of your age, our firm recommends that our clients print and complete this valuable document.  The person(s) completing the document should make known its whereabouts to whoever will be responsible for accessing the information during an emergency or at time of death.
